Registration and Forms
If you have not already done so, please be sure to turn in copies of all the forms below. Not sure if you've already turned it in? Contact our Admissions Office.
- Registration Form with a Passport-Sized Photograph of the student
- Immunization Records and the New Student Health Form should be submitted ASAP directly to schoolnurse@dominicanacademy.org. All students are required to have a full physical exam within 12 months of their start-date at Dominican Academy. All students are required to be vaccinated against COVID-19 to attend school. For students with allergies or who require medication to be administered at school, please request additional forms from Nurse Jessie Lawler.
Technology Requirements
Each student must have their own personal computer or device to bring to school each day and to use at home. Please click here for more information about the recommended devices for students.
Please note that, no matter which device you choose for your daughter, it must have the capacity to run the essential software programs provided by D.A. As part of the Dominican Academy experience, we provide each student with Microsoft Office 365, the Adobe Creative Cloud Suite, Turnitin, a New York Times All-Access pass, and Google Classroom. The primary purpose of Google Classroom is to streamline the process of communication, sharing files, texts, assignments, and notifications between teachers and students.

Summer Homework
SUMMER ELA Homework: All incoming 9th grade students are expected to complete summer assignments in ELA online. Access information will be sent to your D.A. student email.
SUMMER READING: All students must also purchase and read five texts in preparation for their Honors Introduction to Literature course. Please read this letter for instructions and click here to access the reading list.
MATH PLACEMENT EXAM: Students currently taking Algebra I who wish to be considered for Geometry in the 9th grade at D.A. must sit for the D.A. Math Placement Test which will take place in person at D.A. on Thursday, June 2nd from 4 to 6 PM. There is no make-up exam and the Regents cannot be used as a stand-in for the placement test.
Tuition Payments
When do I have to pay tuition?
The first tuition payment date is July 15, 2022. As a reminder, all payments going forward will be made directly through FACTS, our online tuition management system. If you have not yet done so, it is very important that you create an account with FACTS, our tuition management system ASAP, no later than June 15, 2022. Once you have created an account, the Finance Office can manually enter your previous payments and any tuition reductions. Click here for the FACTS Tuition Management worksheet.
Uniforms
We partner with Flynn O’Hara Uniforms, a family-owned business, for all student uniform needs. All students are required to wear the uniform.
Items may be purchased either online or in-store. To purchase online, visit the Flynn O'Hara website and enter our school name to view and order uniform items. To purchase in-store, use this store locator to find a Flynn O'Hara location near you. Contact Ms. Metzler with questions or for recommendations. Uniforms ordered online before August 1, 2022 will be mailed to your home and will arrive in time for the start of school.
UNIFORM REQUIREMENTS: The blazer is required for all students. There is a winter and spring uniform. The winter uniform consists of a blue or white long or short sleeved Oxford shirt, the blackwatch plaid kilt and/or navy uniform slacks, and dark tights. The spring uniform consists of either a white Oxford shirt or school polo, pinfeather blue kilt or navy slacks, and white knee high socks or flesh colored stockings. The winter uniform is appropriate to wear year round and the spring uniform is worn in September, May, and June. Students can wear the school blazer, sweater, cardigan, vest, or quarter zips with the uniform. Hoodies are not considered appropriate uniform attire. Please note that the hem of any uniform skirt must be no higher than 2" above the knee.
SHOES: There is no set uniform shoe. Uniform shoes can be any solid-print brown, black, or navy close-toed shoe with a heel no higher than 1.5". Shoes that reach/cover the ankle, canvas shoes, and any athletic shoes are not appropriate to wear with the uniform. Many students like to wear ballet flats, oxfords, penny loafers, or sperrys. Sneakers are never appropriate uniform attire and can only be worn with an injury and doctor’s note.

Athletics
Tryouts for the 2022-2023 season are fast-approaching! Soccer, Volleyball, and Track will have tryouts and practices in August before school. Please click here to view information about tryouts, coaches, and contacts.
Supply List
Back to School FAQs
When is the first day of school? Orientation for the Class of 2026 and incoming transfer students will take place beginning on September 7th. Students are expected to be present on all Orientation days.
What should I bring? You should bring something to write with, a notebook, your electronic device, a snack, and some pocket money ($10) for the First Day of School. On that day, you will receive your planner, lots of forms, and your class schedule.
What do I wear? Expect to wear your full Summer or Winter Uniform. All students must wear a mask or face covering whenever they are in the school building, and should practice good hand hygiene and social distancing whenever possible.
What supplies do I need? Check above for a list of required supplies for Freshmen.
How will I get my uniform? Uniform orders should be placed directly through the Flynn O'Hara website. Orders will be mailed to your home in August before the beginning of school. If you would like to view uniform items or have questions about your order, please contact Flynn O'Hara Uniforms.
When will I get my new student ID? All new students will receive a D.A. ID card a few weeks into the new school year. This can be used for entrance into the building, checking books from the library, admittance to school dances, etc. and should be kept safe and secure. School picture day will take place on September 12, 2022. More details to follow.
