Required Forms

Any new or returning student who wishes to participate in D.A. athletics for the 2023-2024 seasons must complete the following forms.

• Before Try-Outs--Athletic Registration Form: Required for all new and returning student-athletes. This must be completed and submitted before try-outs. Students who have not completed this form will be unable to participate in try-outs. Click here for the registration form.
• After Making a Team(s)--Athletic Payment Form: Required for all students who have tried out and been accepted onto a team(s). This must be completed as soon as possible. Students who fail to submit this form will be unable to participate in practices or games.
• Interval Health History Form: Required for all student-athletes. This must be completed by a parent/guardian and submitted to schoolnurse@dominicanacademy.org no sooner than 30 days prior to the first try-out. Failure to do so will result in the student being unable to participate in any athletic activities. Check here for the Interval Health History Form.
• Child & Adolescent Health Examination Form: Required for all student-athletes. The annual health exam must be signed by the student-athlete's health care practitioner attesting that she has had a complete health exam within the last 12 months and is medically fit to participate in athletics without restrictions.
