Add-Drop Form

ADD Deadlines: 

Students may request to add a course if space is available up until the 10th day of school, using this form, and with unanimous approval from a parent/guardian, the current instructor, Guidance & the Principal. 

 

DROP Deadlines:

Students may drop a class from their schedule up until the 15th day of school (and maintaining sufficient credits); the submission of this form, a typed 200 word personal statement explaining why attached;  a check for $75 payable to Dominican Academy; and with unanimous approval from a parent/guardian, the current instructor, Guidance, Dean & the Principal.
